AKwaaba!

Design forms visually, collect data offline, and export submissions — all from your browser. Built on the XLSForm standard used by WHO, UNICEF, and thousands of field teams worldwide.

Everything You Need for Field Data Collection

A complete form-building toolkit — from design to data export — that works even without internet.

Visual Form Designer

Drag-and-drop question builder with a live canvas. Add text fields, selects, dates, GPS, photos, calculated fields, groups, and repeating sections.

Offline-First Collection

All forms and submissions are stored in your browser's IndexedDB. Collect data in the field with zero connectivity — no server required.

XLSForm Question Types

Supports the full XLSForm type palette: text, integer, decimal, range, select_one, select_multiple, date, time, geopoint, image, audio, note, calculate, and more.

Skip Logic & Validation

Add relevance conditions to show/hide questions based on previous answers. Set constraints to validate input ranges, patterns, and required fields.

CSV & JSON Export

Export your collected data as CSV for spreadsheet analysis. Import and export form definitions as JSON to share designs across devices.

Mobile-Ready

Responsive design with bottom navigation, touch-friendly controls, and camera/GPS capture. Works on any phone browser — no app install needed.

How It Works

Three simple steps from form design to actionable data.

1

Design Your Form

Open the Form Builder. Add questions from the palette — text inputs, dropdowns, GPS, photos, groups, repeats. Configure labels, hint text, skip logic, and validation rules.

2

Collect Data

Switch to the Collect tab and select your form. Fill out entries in the field — online or offline. Capture GPS coordinates, take photos, and submit. Everything saves locally.

3

Review & Export

View all submissions in the Data tab. Filter by form, inspect records, and export to CSV for analysis in Excel, Google Sheets, or any data tool.

Pre-Built Forms

Ready-to-use clinical and public health data collection forms, deployed and field-tested.

Built on the XLSForm Standard

Our form builder follows the XLSForm specification — the same standard used by ODK, KoBoToolbox, and Ona. This means your forms use familiar concepts: survey sheet (question types, names, labels, constraints), choices sheet (option lists), and settings sheet (form_id, title, version).

Supported Question Types

text · integer · decimal · range · select_one · select_multiple · date · time · datetime · geopoint · image · audio · file · note · acknowledge · calculate · hidden

Form Logic

required · constraint · relevant (skip logic) · default · read_only · calculation · appearance (minimal, columns, likert) · begin_group / end_group · begin_repeat / end_repeat
